Abstract




Distraction injuries in the occipitum cervical junction can cause atlanto-occipital joint or atlanto-axial joint dissociation. On these unstable injuries, diagnosis and rapid reduction treatment should be priority in order to avoid devastating results.
We present the case of a 31-year-old male, who was victim of a car crash accident, and delayed mobilization to a health care service. Clinically the patient presented 7G ECG, isochoric pupils, norm reactive pupils, and left brachiocrural hemiparesis. The diagnosis of dissociation of C1 - C2 was made, and 48 hours later, he presented epidural hematoma C3-T1 with symptoms.


With two-stage surgical resolution and sequential improvement in general condition.
