Lesiones osteocondrales del astrágalo. Tratamiento individualizado y descripción de técnica quirúrgica

Abstract
Osteochondral defects of the talus are poorly recognized lesions thata are difficult to diagnose. The therapeutic approach to be followed will depend on the complexity of the lesion, the size and displacement of the defect, being able to opt for a conservative versus surgical management. Objective: to describe the clinical and radiological characteristics of osteochondral lesions of the talus and the surgical technique used for their resolution. Material and methods: Report of 4 clinical cases, evaluated in the Traumatology and Orthopedics area of Hospital Metropolitano Quito between January 2021 anda January 2022, characterized by chronic ankle pain secondary to a traumatic event, diagnosed with osteochondral lesion of the talus and surgically treated. Conclusions: Osteochondral defects of the talus (LOA) are frequent but underdiagnosed. The treatment will depend on the complexity of the lesion, however, this is still a challenge due to the poor regenerative properties of articular cartilage and the different injury patterns. More studies, and long-term results are required to guarantee the success and efficacy of one surgical technique over another.
